Where do hands go? An audit of sequential hand-touch events on a hospital ward

背景
病棟の気づかれない場所に病原体のリザーバが存在している可能性があり、洗っていない手指を介して患者に伝播をもたらす継続的なリスクとなる。
目的
表面と患者間での微生物伝播の可能性を監視すること、および患者ケア中の病原体の伝播経路について理解を深めること。
方法
30 分間の調査を夏季と冬季に合計 40 回実施し、手指接触行動を単盲検的に監査した。手指接触行動は以下のカテゴリに分類した。入室時の手指衛生、患者の隣接部位への接触、患者への接触、医療器具への接触、退出時の手指衛生、および病室外の部位への接触。
結果
合計 104 回の入室があり、このうちの 77 回は臨床職員(59 回は看護師、18 回は医師)、21 回は病院家事労働者、1 回は薬剤師、5 回は親族によるものであった。入室前後の臨床職員の手指衛生遵守率は 25%(38/154)であり、夏季の 20 回の監査時(47%、95%信頼区間[CI]35.6 ~ 58.8)のほうが、冬季の 20 回の監査時(7%、95%CI 3.2 ~ 14.4、P < 0.0001)より遵守率が高かった。半数以上(58%、45/77)の臨床職員に患者との接触があった。臨床職員の手指洗浄率は、患者との接触前(オッズ比[OR]3.44、95%CI 0.94 ~ 16.0、P = 0.059)および患者の周辺部位との接触前(OR 6.76、95%CI 1.40 ~ 65.77、P = 0.0067)に高かった。臨床職員の約半数(48%、37/77)が患者のカルテに、25%がベッドに接触していた。手指が接触する頻度が高かった器具類は、病室内では点滴(30%)および血圧計スタンド(13%)、病室外ではコンピューター(26%)、カルテ台(23%)、および電話(21%)であった。
結論
単盲検的な観察による手指衛生遵守率は依然として低い。高い頻度でみられる手指と表面との相互作用を理解することにより、清掃を行うべき部位が明確になると考えられる。
