Prevalence survey of healthcare-associated infections in Argentina; comparison with England, Wales, Northern Ireland and South Africa

背景
有病率調査は、医療関連感染症（HCAI）の発生と分布を系統的に観察することによって、適切な対策を講じることを可能にするための方法である。
目的
本有病率調査の目的は、各国で検証された方法を用いてアルゼンチンにおける HCAI 有病率を初めて明らかにするとともに、国際的なベンチマークとなるデータを提供することである。
方法
ブリテン諸島の急性期病院を対象とした第 3 回 HCAI 調査で英国病院感染学会が採用した方法と同一の方法を用いて、アルゼンチン 23 州のうち 7 州の 39 病院を対象として、2008 年に HCAI 有病率調査を実施した。収集したデータを英国ベルファストの北アイルランド医療関連感染調査センター（Northern Ireland Healthcare-Associated Infection Surveillance Centre）で処理し、解析を行った。
結果
合計 4,249 例の患者を調査した。このうち 480 例が 1 回以上の HCAI を経験しており、有病率は 11.3％であった。男性の有病率は 13.6％、女性は 9.0％であった。頻度の高い HCAI は肺炎（3.3％）、尿路感染症（3.1％）、手術部位感染症（2.9％）、原発性血流感染症（1.5％）、および軟部組織感染症（1.2％）であった。手術を受けた 1,027 例の患者の手術部位感染症有病率は 10.2％であった。メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）保菌率は 1.1％であり、全 HCAI 分離株の 10.0％を占めていた。アルゼンチンの調査結果は、イングランド、ウェールズ、北アイルランド、および南アフリカと比較して高い HCAI 有病率を示した。
結論
本調査は、医療資源の優先順位を明確にするうえで有用であり、保健行政機関や病院に対して HCAI 抑制のための継続的な取り組みに関する情報を提供するものであると考えられる。
