ベッド柵の清掃レベルおよび黄色ブドウ球菌(Staphylococcus aureus)伝播に対する表面塗装・処理の影響
Effect of surface coating and finish upon the cleanability of bed rails and the spread of Staphylococcus aureus

背景
患者の隣接環境の細菌リザーバは医療関連感染を媒介することが多い。
目的
プラスチック製のベッド柵上の細菌数は塗装されたスチール製上よりも多いという従来の知見を、実験室内での研究により確認すること。
方法
種々の合成汚染物中に懸濁した黄色ブドウ球菌(Staphylococcus aureus)を 6 種類のベッド柵表面に接種した。好気性細菌コロニー数および ATP バイオルミネッセンス法により、マイクロファイバークロスと抗菌ワイプの清掃効果を評価した。さらに、指先と各ベッド柵間の黄色ブドウ球菌の伝播率を調べた。
結果
抗菌ワイプでは、すべての柵上の細菌数が検出可能レベル未満にまで減少したが、有機汚染物の除去についてはマイクロファイバークロスよりも効果が低かった。汚染除去が相対的に容易な表面では、接触による黄色ブドウ球菌の伝播率が高かった。消毒が不十分な柵では、交差伝播のリスクが高かった。汚染物がない場合は、指先から柵への細菌の伝播率は 38%から 64%、柵から指先への伝播は 22%から 38%であった。表面の素材および粗さの程度は、清掃レベルおよび伝播率の重要な決定因子であった。しかし、有機汚染物の存在は、素材や処理にかかわらずすべてのベッド柵からの細菌伝播に影響した。
結論
ベッド柵は高度に汚染されることがある。抗菌ワイプによる定期的な拭き取りは、患者の隣接環境の細菌数を低く維持するための費用対効果の高い方策であると考えられる。交差伝播のリスクを最小にするためには、清掃プロトコールを検証して微生物および非微生物による表面汚染を効果的に除去できるようにしておく必要がある。
