小手術（minor surgical procedure）および小範囲侵襲的介入（minimal access intervention）を実施する施設の要件に関するガイドライン

近年の医療提供体制は大きく変化しており、例えば、かつては急性期病院が担っていた外科診療の一部はプライマリ・ケア施設や日帰り手術専用施設で実施されるようになっている。インターベンショナルラジオロジーや心血管インターベンションの領域が発展したことから、上述の医療施設で実施される手技は一層、多様かつ複雑になった。こうした変化に伴い、このような外科手技が実施される施設の基本的な物理的要件を、感染の予防・制御の見地から明確にする必要がある。著者らは Healthcare Infection Society の支援の下、新規施設の設計や既存施設の改修のための勧告を作成した。その作成にあたっては実用的かつ実施可能な提言を行うため、最良事例、現行のエビデンス、可能な場合は他のガイドライン、およびエキスパートコンセンサスを活用した。また、小範囲侵襲的介入（minimal access intervention）※と小手術（minor surgical procedure）※※の定義も試みた。インターベンショナルラジオロジーを含む小範囲侵襲的介入については、新規施設では 1 時間あたり 15 回の機械的換気を行うべきであるが、小手術では自然換気で十分とした。すべての手技に際してはチェックリストを使用し、操作者は適切な訓練を受ける必要がある。正確な術後感染率を明らかにするために、前向きサーベイランスも必要である。最後に、このガイドラインが今後、繰り返し利用されるものとするために必要なエビデンスの基礎を築くためには、適切な応用研究が求められる。
