Outbreak of adenovirus serotype 8 conjunctivitis in preterm infants in a neonatal intensive care unit

背景
早産児のアデノウイルス角結膜炎のアウトブレイクの報告はまれである。トルコの大学病院の新生児集中治療室で、1 月 15 日から 2 月 25 日にアデノウイルス結膜炎のアウトブレイクが発生した。
目的
アウトブレイクの拡大、調査、および管理について述べること。
方法
PCR 法により、14 サンプルからアデノウイルス 8 型が同定された。リスク因子を明らかにするために症例対照研究を実施した。
結果
早産児 15 例、医療従事者 5 例、および新生児の親 4 例に流涙、眼の腫脹や充血などの結膜炎の徴候が認められた。アデノウイルス結膜炎の最も重要なリスク因子は、未熟児網膜症の検査であった（オッズ比 17.5、95％信頼区間 1.9 ～ 163.0、P ＝ 0.012）。未熟児網膜症の検査に使用された開瞼器が患者ごとに滅菌されておらず、これが汚染源であることが判明した。
結論
バリアプリコーション、手指衛生、開瞼器の滅菌、患者の他科への移動の待機、および感染した医療従事者の最低 15 日間の出勤停止などの対策により、アウトブレイクは制御された。
