Ventilator-associated pneumonia due to meticillin-resistant Staphylococcus aureus: risk factors and outcome in a large general hospital

背景
任意抽出された人工呼吸器関連肺炎（VAP）患者におけるメチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）のリスク因子および転帰への影響に関するデータは限られている。
目的
大規模教育施設における MRSA による VAP の素因および転帰を評価すること。
方法
当院の成人集中治療室 3 室で 4 年間にわたる前向き研究を実施した。MRSA による VAP 患者を他の細菌による VAP 患者と比較した。
結果
合計 474 件の細菌性 VAP エピソードを収集した。MRSA による VAP（111 件）と他の細菌による VAP（363 件）との比較で有意差が認められた因子は、年齢中央値（68 歳対 62 歳）、APACHE II スコア中央値（12 対 11）、脳神経外科手術（5.4％対 13.8％）、腹部手術（35％対 19％）、今回の入院での VAP 発症前の抗菌薬投与歴（82.9％対 64.5％）とイミペネム投与歴（24％対 11％）、および胸水（12％対 5％）であった。交絡因子で補正した多変量解析による MRSA の独立リスク因子は、APACHE II スコア高値、抗菌薬投与歴、および胸水であった。治療および転帰に関して MRSA による VAP と他の細菌による VAP との比較で差が認められた因子は、不適切な経験的治療（70％対 53％）、1 エピソードあたりの抗菌薬の費用の中央値（974 ユーロ対 726 ユーロ）、および院内死亡率（60％対 47％）であった。しかし多変量解析からは、MRSA は死亡の独立リスク因子であることは示されなかった。
結論
MRSA は、VAP の原因として高頻度にみられる。基礎疾患の重症度が高い場合は、VAP 患者の死亡率が高くなる。
