A two-stage algorithm for Clostridium difficile including PCR: can we replace the toxin EIA?

クロストリジウム・ディフィシル（Clostridium difficile）感染の診断のための 2 段階、3 試験からなるアルゴリズムを評価した。便検体に対する酵素免疫法により、C. difficile の一般的な抗原であるグルタミン酸脱水素酵素（G）と毒素 A/B（T）を調べた。G と T の結果が一致しない検体は、PCR 法による毒素 B 遺伝子（P）の検出を行った。本アルゴリズムでは毒素 A/B の検出が可能な患者を迅速に特定できたが、毒素産生 C. difficile を排出しているが毒素 A/B 産生レベルが検出限界以下（G^＋T^－P^＋）である患者が多数存在することが判明した。結果が G^＋T^＋ の患者の平均白血球数は、G^＋T^－P^＋ である患者と比較して多かった。
