従来の質問票および電子質問票を用いた小児患者に対する病院関連感染に関する退院後追跡調査★
Post-discharge follow-up of hospital-associated infections in paediatric patients with conventional questionnaires and electronic surveillance

背景
ウイルス感染は小児患者における病院関連感染(HAI)の原因として高い頻度で認められ、その多くは退院後に判明する。
目的
HAI に関する追跡調査の方法として、従来の調査法と電子的調査法の有益性と費用を分析すること。
方法
回答率、患児 1 例に要する時間、および医療施設に要した費用を、従来の調査法と電子的調査法(ショートメール、電子メール、電話)とで比較した。
結果
2001 年から 2003 年に合計 1,927 例が従来の追跡調査に参加し、このうち 1,175 例(61%)が質問票に回答した。2005 年から 2007 年の電子的調査の期間中に 2,309 例が病院内で調査を受け、このうち 1,940 例(84%)が退院後の情報について回答した。医療従事者が患児 1 例あたりに要した時間は、従来の追跡調査では 33 分、電子的追跡調査では 13 分であり、患児 1 例あたりの総費用はそれぞれ 15.07 ユーロ、13.61 ユーロであった。電子的追跡調査では、毎年の費用の削減率は 17.1%に達した。HAI 発生率は、従来の調査を行った期間では 8.4%、電子的調査を行った期間では 12.2%であり、大半の症例では退院後に症状が発現した。
結論
電子的なデータ収集は、HAI の継続的な追跡調査のための簡便な方法であり、回答率が高く費用も安価であった。

電子サーベイランスの経済的効果をうたっているが、デジタルデバイダー対策はどうしているのだろうか? 携帯電話や電子 pad の普及とともに、こうした手法は今後主流となるだろう。
