Hospital-associated infections in children: a prospective post-discharge follow-up survey in three different paediatric hospitals

背景
小児科病棟および小児病院に入院中の小児におけるウイルス性病院感染(HAI)の発生率は、1%から 24%の範囲と報告されている。このようなばらつきの原因は不明である。
目的
小児病院 3 施設のウイルス性 HAI の発生率およびリスク因子を評価すること。
方法
フィンランドおよびスイスの感染症病棟 1 施設と一般小児病棟 3 施設で、2 年間にわたりデータを前向きに収集した。入院中および退院後 1 週間の感染を記録した。多変量ロジスティック回帰分析により、各病棟のそれぞれの HAI のリスク因子を調べた。
結果
合計 5,119 例が入院した。HAI の全発生率は 12.2%であり、患児の 2.4%は病院内で HAI を発症し(最も頻度が高いのは胃腸炎)、9.8%(95%信頼区間[CI]8.9% ~ 10.8%)は退院後 72 時間以内に発症した。HAI 発生率は病棟によって 5.8%から 17.1%とばらつきがあり、その頻度は、相部屋が多く、ウイルス性の感染症を対象とした積極的なコホーティングを実施していない一般小児病棟で最も高かった。相部屋(オッズ比[OR]5.45、95%CI 2.44 ~ 12.2、乳児を対象とした一般小児病棟)、長期の入院(1 日あたりのOR 1.42、95%CI 1.20 ~ 1.67、感染症病棟)、および低年齢(1 歳あたりの OR 0.71、95%CI 0.51 ~ 0.98、1 歳以上の小児を対象とした一般小児病棟)では、入院中の HAI リスクが高かった。
結論
小児病棟におけるウイルス性 HAI の多くは退院後に発症していた。個室病床は HAI 予防、特に呼吸器系ウイルス伝播の予防に効果的であると考えられた。また、感染症に罹患した患児の別室へのコホーティングも有益と考えられた。
