Trend of healthcare-associated infections in children: annual prevalence surveys in a research hospital in Italy, 2007-2010

背景
2007 年から 2010 年に、イタリアで最大規模の 3 次小児病院で医療関連感染（HAI）の年次有病率調査が実施された。この期間中に、強化隔離策、侵襲的処置に関するケアバンドルの適用、世界保健機関（WHO）の多面的戦略を用いた手指衛生の促進、および適切な抗菌薬使用の促進など、HAI 予防の改善のための措置が講じられた。
目的
HAI 率に対するこれらの対策の効果を明らかにすること。
方法
合計 1,506 例の患者を調査対象とした。患者の人口統計学的特性、人工呼吸器の使用、調査前 48 時間以内の中心ラインおよび尿路カテーテルの留置、および調査前 30 日以内の手術に関する情報をカルテから抽出した。HAI の種類と発症日、および微生物学的データを記録した。単変量および多変量ロジスティック解析を用いて、HAI 率の経時的変動、および HAI リスクに対する病棟の種類と患者の特性の影響を評価した。
結果
HAI 有病率（7.6％から 4.3％へ）、および全 HAI 有病者数の割合（100 例あたり 8.6 件から 4.3 件へ）は有意に低下した（P ＜ 0.001）。HAI リスクの上昇と独立して関連する因子は、集中治療室への入室、30 日を超える入院、侵襲的器材の使用、および年齢が 6 ～ 11 歳であることであった。
結論
本研究の HAI 予防戦略は、入院中の小児の感染を減少させるうえで効果的であった。定期的な有病率調査は、HAI の頻度のモニタリング、医療従事者の認識の向上、および効果的な感染制御の確立のための効果的なツールである。
