最終清掃後の集中治療室の臨床環境表面における多剤耐性菌の生菌を含むバイオフィルムの存在

背景
近年は表面清掃や手指衛生プログラムの重要性が認識されているにもかかわらず、病院環境からの多剤耐性菌の分離は依然として続いている。菌体外高分子物質中に細菌を含むバイオフィルムは、洗浄剤や消毒薬に対する抵抗性が増大しているため除去が困難であり、感染源となり得る浮遊細菌を環境中に持続的に放出する。
目的
多剤耐性菌のリザーバが環境中にバイオフィルムとして存在しているかどうかを明らかにすること。
方法
集中治療室(ICU)の最終清掃後に器具や備え付けの備品を無菌的に取り出し、培養検査および走査型電子顕微鏡検査(SEM)を行った。サンプルをトリプトンソーヤブイヨン 5 mL 中に採取し、5 分間の超音波処理後、ウマ血液寒天、Brillance MRSA Agar、および Brilliance VRE Agar の各平板培地で培養した。SEM のサンプルは、3%グルタルアルデヒドとヘキサメチルジシラザン(HMDS)で固定し、金スパッタリング後に電子顕微鏡で観察した。
結果
滅菌材料容器、半透明のプラスチック製ドア、ブラインドのひも、およびシンクのゴム部にはバイオフィルムが視覚的に確認されたが、カーテンには菌体外高分子物質のみが認められた。3 か所のサンプル培養で生菌が増殖し、このうちブラインドのひもとカーテンからは MRSA の増殖がみられた。
結論
多剤耐性菌を含むバイオフィルムは最終清掃後にも ICU の臨床環境表面に残存しており、現行の清掃方法はバイオフィルム形成の制御には不十分であることが示唆される。バイオフィルム内に多剤耐性菌が保護されて存在することが、病院環境内に多剤耐性菌が残存する機序であると考えられる。

要するに徹底した環境浄化が最終消毒には求められるということである。
