背景
スコットランドでは、メチシリン耐性黄色ブドウ球菌(meticillin-resistant Staphylococcus aureus;MRSA)のスクリーニングの有効性に関する医療技術評価(HTA)モデルに基づき、発色酵素基質寒天培地を用いたユニバーサルスクリーニングが有効性と費用の点で望ましい選択肢であることが示唆されている。
目的
1 年間のパイロット研究により、このモデルの妥当性を検証すること。
方法
国民保健サービス(NHS)スコットランドの急性期病院 6 施設において、入院 81,438 件を対象とした 1 年間にわたる MRSA スクリーニングの大規模前向きコホート研究を実施した。アウトカム(MRSA の保菌率および感染率)の多変量解析を行い、スクリーニング実施前後の動向を比較した。
結果
保菌率は、当初は 5.5%であったが 12 か月後は 3.5%に低下した(P < 0.0001)。保菌は、患者 1 例あたりの入院回数、入院した診療科、年齢、および入院前の滞在場所(自宅、他の病院、または介護施設)と関連していた。MRSA 感染歴または保菌歴のない患者の全入院事例における検査陽性率は約 2%であった。入院時スクリーニング陰性患者と比較した感染症発症率は、入院時スクリーニング陽性かつ過去に陽性歴がない患者では 12 倍、入院時スクリーニング陽性かつ過去に陽性歴がある患者では 18 倍であった。1 年間の研究期間中に MRSA 感染症も有意に減少した(全体で 1,000 入院患者日あたり 7.5 件、P = 0.0209)。
結論
本研究で特定された MRSA 保菌および感染のリスク因子から、全患者を対象とした臨床的リスク評価がMRSA のスクリーニングに有用である可能性が示唆される。
監訳者コメント:
積極的な監視培養を全患者に対して実施して、すり抜けをなくすことによる感染予防効果を証明したものである。我が国での同様なトライアルが望まれる。
