血液腫瘍内科における給水システム関連の非結核性抗酸菌菌血症アウトブレイク★

非結核性抗酸菌は広く存在する環境微生物であるが、感染症の起因菌となることはまれである。当院の血液腫瘍内科における非結核性抗酸菌菌血症アウトブレイクの臨床的、微生物学的、および疫学的調査と、その後の管理について報告する。2007 年 10 月から 2008 年 7 月に、血液悪性腫瘍の管理を受けていた患者 5 例に発熱と全身倦怠感が発現した。血液培養によりマイコバクテリウム・ムコゲニカム(Mycobacterium mucogenicum)(4 例)とマイコバクテリウム・ネオオーラム(Mycobacterium neoaurum)(1 例)が同定された。感染源を特定するために環境、特に給水システムを調査し、複数の水サンプルを常法に従って培養した。非結核性抗酸菌は病院の給水システムからも分離された。中心静脈カテーテル(CVC)を抜去し、抗菌薬を投与することにより患者は回復した。これらの患者のさらなる非結核性抗酸菌菌血症を予防するため、環境対策および CVC ケア改善策を導入した。これらの対策にもかかわらず水供給システム中の非結核性抗酸菌の存在は持続したが、新規症例は確認されなかった。非結核性抗酸菌は一般的な環境微生物であり、給水システムからの除去は困難であると認識されている。今回の非結核性抗酸菌菌血症アウトブレイクの進入路は CVC であると考えられ、CVC ケアプロトコールの改善により免疫低下患者群におけるさらなる感染拡大の予防に成功した。
監訳者コメント:
CVC ケアの変更点とは、ライン接続システムの簡素化(より接続部位個数の少ないものに変更)と、刺入部のシャワー時被覆材使用(以前ははがしてシャワーしていた)である。環境の病原体自体をなくすことは困難であり、ケアの改善によって感染防止を図るという方向性は妥当である。
