Characteristics of hospital patients colonized with livestock-associated meticillin-resistant Staphylococcus aureus (MRSA) CC398 versus other MRSA clones

Clonal complex（CC）398に関連するメチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）は、家畜や家畜に曝露したヒトの新興病原菌である。これまで MRSA CC398 は医療環境中の MRSA の拡散および疾患負荷との関連は比較的低いと考えられていた。本研究の目的は、MRSA CC398 保菌患者とその他の MRSA クローンの保菌患者における人口統計学的および臨床的特性の相違が、伝播および感染率の相違に寄与しているかどうかを評価することである。2008 年から 2009 年に当大学病院に入院した全 MRSA 保菌患者を対象として、年齢、性別、入院期間、診断、および診療内容を評価した。各患者から得た最初の MRSA 分離株の黄色ブドウ球菌プロテイン A 遺伝子（spa）のタイピングを行った。spa 遺伝子型 CC398 を示した MRSA（MRSA CC398）の保菌・感染患者を、他の MRSA クローン（MRSA 非 CC398）を認めた患者と比較した。MRSA CC398 患者と MRSA 非 CC398 患者では、年齢（53 歳対 59 歳）、平均入院期間（8 日対 13 日）、および集中治療室入室患者の割合（12％対 17％）が有意に異なっていた。診断および診療内容の平均件数と種類も両群の患者間で有意に異なっていた。医療環境における MRSA CC398 の伝播および感染率が相対的に低いことは、少なくともその一部は患者特性の差によって説明できると考えられる。
