血管内留置カテーテルの防腐剤含有ロック溶液の微生物定着防止効果

防腐剤含有ヘパリンロック溶液によりカテーテル関連感染が予防できるかについて、論文発表されているエビデンスはほとんどない。しかし、防腐剤含有溶液によるフラッシングが新生児に有害な作用を及ぼすことが示されており、多くの病院がその使用を完全に中止している。カテーテル関連感染の発生および防腐剤含有静脈内留置カテーテルロック溶液の使用について、St. Jude Children’s Research Hospital(SJCRH)における 1982 年から 2008 年の感染制御記録を調査した。さらに、防腐剤であるパラベン(0.165%)またはベンジルアルコール(0.9%)を含有するヘパリンロック溶液および 70%エタノールの抗菌活性を、表皮ブドウ球菌(Staphylococcus epidermidis)、黄色ブドウ球菌(Staphylococcus aureus)、大腸菌(Escherichia coli)、バチルス・セレウス(Bacillus cereus)、緑膿菌(Pseudomonas aeruginosa)、およびカンジダ・アルビカンス(Candida albicans)を対象として評価し、ヘパリン含有および非含有の防腐剤非含有生理食塩液と比較した。各試験液を 35℃で 0、2、4、および 24 時間曝露後に菌の増殖を評価した。浮遊(遊離)菌と固着(バイオフィルム形成)菌に対する防腐剤の効果を MBEC アッセイにより評価した。感染制御記録から、カテーテル関連感染の増加がみられた 2 つの期間は、SJCRH で防腐剤非含有ヘパリンが使用された 2 期間と一致することが判明した。防腐剤含有ヘパリン溶液は、浮遊性および固着性の 6 種類の菌種のいずれに対しても有意な抗菌活性を示した。エタノールは最大の抗菌活性を示し、特にインキュベーション時間が短い場合に顕著であった。防腐剤であるパラベンまたはベンジルアルコールを含有するヘパリンロック溶液と 70%エタノールは、カテーテル関連感染に関与することが多い浮遊菌および固着菌に対して、有意な抗菌活性を示した。これらの知見と著者らの感染制御の経験から、カテーテル関連感染を減少させるために、新生児期以外の患者では静脈内ロック溶液に防腐剤を使用することが支持される。
