英国の地域病院における MRSA サーベイランス：MRSA 臨床分離株の検査は MRSA 菌血症の検査よりも有用である

英国でメチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）菌血症の義務的サーベイランスおよび半減目標が導入され、これにより大半の病院で菌血症が減少した。しかし、ピーターボロ・スタンフォード病院 NHS ファンデーショントラストでは、MRSA 菌血症がすでにまれなイベントとなっていたため、減少の確認が困難であった。著者らは有病率の低い病院における菌血症モニタリングの有効性に疑問をもち、本研究では介入の有効性を評価するため、菌血症検査の精度を全臨床分離株の検査（菌血症を除く創傷、喀痰、尿など）と比較した。6 年間の研究期間中に臨床検体中の MRSA は有意に減少し、スクリーニングにより新規に MRSA 保菌者が特定されたものの MRSA 菌血症有病率は低値が持続した。現在、英国のほぼ全域で MRSA 菌血症有病率は低値まで減少しているため、臨床分離株の検査は介入の有効性を評価するうえでより有用であると考えられる。
