大腸菌（Escherichia coli）菌血症：どの程度予防可能か？

2011 年 6 月から菌血症の報告義務の範囲が拡大され、大腸菌（Escherichia coli）が対象となった。本研究の目的は、メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）感染率低下の成功が、大腸菌にも同様に可能であるかどうかを検討することである。2010 年に著者らのトラストで発生した大腸菌菌血症の全症例のデータを精査した。大腸菌菌血症のエピソードは 216 例にみられ、このうち 63％が市中獲得型であった。その原因が予防し得るものであることが特定された患者は 19％のみであり、その半数以上（71％）は尿路カテーテル関連菌血症であった。今後、大腸菌菌血症の減少を目指す際には、これらのデータを念頭に置く必要がある。
監訳者コメント：
大腸菌の菌血症の 3 分の 2 が市中獲得型であるということは、医療関連感染防止対策だけではその制御が難しいということを物語っている。MRSA も以前と異なり市中感染型と思われる感受性パターンのものが散見されるようになり、今後の感染対策のあり方に一石を投じるデータである。
