Burden of meticillin-resistant Staphylococcus aureus colonization and infection in London acute hospitals: retrospective on a voluntary surveillance programme

メチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）は病院感染および市中感染の重要な原因であり、その保菌は感染の前段階であると見なされているが、急性期医療施設での保菌・感染負荷を明確な医療経済の枠内で評価する研究はほとんど行われていない。本稿の目的は、2000 年から 2001 年に任意のサーベイランスプログラムに参加したロンドンの急性期病院トラストにおける MRSA 保菌・感染の有病率と発生率について記述することである。MRSA による保菌、菌血症、およびその他の重要な感染症の発生と罹患の詳細を、病院の感染制御担当者が週報に記入した。参加期間が両年とも十分であった病院で有病率と発生率を算出した。MRSA 新規発生症例の 79％が保菌であり、菌血症は 4％、その他の重要な感染症は 17％であった。2000 年と 2001 年の入院患者の保菌発生率に変化は認められなかった。一方、この期間に保菌有病率の49％の原因不明の上昇がみられた。いずれの月においても、保菌者数は新規保菌者数の 2 倍以上であった。英国の地域医療経済の枠内で保菌と感染の両方について発生例と罹患例を前向きに評価したという点で、この MRSA サーベイランスプログラムは異例であった。他の研究と同様に、保菌の発生率と有病率は、感染の場合を大幅に上回っていた。MRSA の全負荷に占める菌血症の寄与が小さいことや、近年のサーベイランス、スクリーニング、および制御介入を考慮すると、現在の菌血症サーベイランスの偏重は再考するのが妥当であると考えられる。
