Mupirocin resistance is not an inevitable consequence of mupirocin use

ブザンソン大学病院ではメチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）のムピロシン耐性率は低値であり、2004 年の 10％から 2009 年の 3％へと低下傾向にある。この耐性率の低下傾向はムピロシン使用量の減少と並行していた。遺伝子タイピングから、ムピロシン耐性率の低下は MRSA クローンの変化とは無関係であることが示された。ムピロシンの使用量よりもいかに使用するかということが耐性出現に関与しており、慎重に使用すればムピロシン耐性は低レベルに維持されるため、その効果は持続すると考えられる。

監訳者コメント：

本論文の著者らの所属する施設では、ムピロシン軟膏の適用とその使用方法を定め、適切に使用していると考えられる。このことがムピロシン耐性 MRSA の減少に寄与している、というのが著者らの主張である。明確な裏づけはないものの、理論的には理解できる。
