整形外科感染症におけるコアグラーゼ陰性ブドウ球菌のメチシリン耐性★

コアグラーゼ陰性ブドウ球菌(CoNS)やメチシリン耐性株による整形外科感染症は増加していると考えられる。CoNSに感染した整形外科患者を対象として、1995 年 1 月から 2007 年 12 月に 13 年間の後向きコホート研究を実施し、CoNS感染症の長期的傾向およびメチシリン耐性に関連する因子を評価した。CoNS感染症 60 件中 57 件(95%)はインプラントと関連していた。治療終了後の追跡期間中央値は、5.1 年(2.4 ~ 13.8 年)であった。研究期間中に実施された整形外科的処置は 44,237 件で、このうちインプラントは 21,299 件(48%)であった。CoNS関連感染症の累積発生率は、全処置では 0.14%、インプラント関連処置では 0.28%であった。CoNS関連感染症の発生数および累積発生率には有意な変動はみられなかった(カイ二乗検定による傾向検定 P = 0.45 および 0.97)。メチシリン耐性株による感染は 45 件(75%)であった。メチシリン耐性菌の割合は長期間一定であった(傾向検定 P = 0.65)。メチシリン感性株 15 件のうち、原因菌に対する術前予防投与が実施されていたのは 4 件と少なかったが、メチシリン耐性株 45 件中 28 件には不十分な予防投与が行われていた(P = 0.03)。当施設におけるCoNSによる整形外科感染症の累積発生率は低く、また一定であり、ほとんどがインプラントのみと関連していた。過去 10 年間のCoNSにおけるメチシリン耐性株の割合は一定であり、臨床転帰は良好であった。
監訳者コメント:
我が国の病院分離コアグラーゼ陰性ブドウ球菌(CoNS)の8割以上がすでにメチシリン耐性であり、こうした情況を踏まえ我が国でも長期入院例で整形外科的治療を行う際には注意が必要である。
