Effect of systemic antibiotics and topical chlorhexidine on meticillin-resistant Staphylococcus aureus carriage in intensive care unit patients

抗菌薬および生体消毒薬はメチシリン耐性黄色ブドウ球菌（meticillin-resistant Staphylococcus aureus；MRSA）の保菌および伝播に影響を及ぼす可能性があるが、その効果は複合的であり、特に複数の薬剤が使用されている場合に顕著である。本研究の目的は、MRSA を保菌している集中治療室（ICU）入室患者 544 例の入院時と週 1 回の MRSA スクリーニング結果、および抗菌薬と消毒薬の連日の処方データを用いて、これらの薬剤が患者の MRSA 保菌の短期的動態に及ぼす影響を明らかにすることであった。患者の MRSA 陽性（MRSA 保菌の検出あり）と MRSA 陰性（保菌の検出なし）の 2 つの保菌状況間の移行を評価する Markov モデルを使用して、時系列データを解析した。抗菌薬全身投与およびクロルヘキシジン局所使用の同時併用が、これらの 2 つの保菌状況間の移行に及ぼす影響を評価した。スクリーニングにより MRSA 保菌が確認された部位へのクロルヘキシジンの使用により、培養陽性から陰性への移行が増加するとともに、その後の陰性から陽性への復帰もエビデンスは弱いながら減少した。一方、抗菌薬の場合はいずれの種類であっても、 2 つの状況間の移行に影響するというエビデンスはどちらの方向の移行についても弱く、また一貫していなかった。例えば、単変量解析によりキノロン系抗菌薬が 1 日の時間内での MRSA 保菌の陰性化リスクおよびその後の再陽性化リスクの増大と強く関連していることが示されたが、週単位のモデルではこのような効果はみられなかった。これらの知見の一般化が可能であるかどうかを明らかにするためには、同様の試験が必要である。
