本稿では、2009 年に血管外科病棟で発生した高度クリンダマイシン耐性クロストリジウム・ディフィシル(Clostridium difficile)リボタイプ 106 による C. difficile 感染症アウトブレイクについて報告する。C. difficile 感染症症例の定義は、下痢を発現し、C. difficile 毒素陽性かつ他の腸内病原菌陰性の患者とした。培養した菌を Scottish Salmonella Shigella and Clostridium difficile Reference Laboratory(SSSCDRL)に送付し、PCR リボタイピング、抗菌薬感受性検査、および PCR による ermB 検出を行った。患者 9 例の平均年齢は 73 歳(範囲 38 ~ 90 歳)であった。全例がクリンダマイシンとシプロフロキサシンの投与を受けていた。全例が PCR リボタイプ 106 に分類され、高度クリンダマイシン耐性を示した。これらの分離株のうち 5 株については、PCR による ermB 遺伝子検査では増幅は認められなかった。この菌株は、この病棟の患者からはこれまでほとんど分離されていなかった。病棟閉鎖中の最終清掃、感染制御・予防策の強化、および新しい抗菌薬指針の導入により、アウトブレイクの制御を達成した。特記すべき点は、このアウトブレイクは ermB が関与しない高度クリンダマイシン耐性株によって引き起こされたことである。さらにこのアウトブレイクからは、C. difficile 感染症のアウトブレイクは PCR リボタイプ 027 以外のリボタイプによって生じ得ることがあらためて強調される。このアウトブレイクは、クリンダマイシンとシプロフロキサシンの使用、および本環境中での芽胞による交差感染と関連している可能性が高い。厳格な感染制御・予防策の実施、抗菌薬の適正使用、および環境清掃の強化は、このようなアウトブレイクの管理を達成するための主要な要素である。これらの介入後は、メチシリン耐性黄色ブドウ球菌(meticillin-resistant Staphylococcus aureus)の獲得数も大幅に減少した。
