A 12-year review of Staphylococcus aureus bloodstream infections in haemodialysis patients: more work to be done

黄色ブドウ球菌（Staphylococcus aureus）血流感染症（BSI）は、血液透析患者の疾患および死亡の重要な原因である。本研究では、3 次紹介病院の大規模な血液透析施設で実施された、黄色ブドウ球菌 BSI の 12 年間の後向き調査について報告する。全体の黄色ブドウ球菌 BSI 発生率は、100 患者年あたり 17.9（範囲 9.7 ～ 36.8）であった。メチシリン耐性黄色ブドウ球菌（meticillin-resistant S. aureus；MRSA）BSI 発生率は、100 患者年あたり 5.6（範囲 0.9 ～ 13.8）であった。黄色ブドウ球菌 BSI の全エピソードのうち 11％に感染性合併症が発生し、最も頻度が高いのは感染性心内膜炎であった（7.6％）。黄色ブドウ球菌が血液中から分離されてから 30 日以内に死亡した患者は 10％であった。大半の黄色ブドウ球菌 BSI 症例（83％）は血管カテーテルと関連していた。すべての血液透析部門は、動静脈瘻のような低リスクの血管アクセスを用い、血管内カテーテルの使用を減らすことを優先すべきである。この脆弱な患者群の疾患および死亡の減少を図るため、代替血管アクセスを確保できない場合は、カテーテル関連感染症のリスク低下のための介入を実施すべきである。
