Surgical site infection after central venous catheter-related infection in cardiac surgery. Analysis of a cohort of 7557 patients

本研究の目的は、手術部位感染症（SSI）の発生と中心静脈カテーテル関連感染の存在との関連を明らかにすることである。ルーアンの大学病院の胸部心血管外科では、1997 年以降、すべての院内感染（肺炎、SSI、中心静脈カテーテル関連感染を含む）について前向きの疫学調査を行っている。1997 年から 2007 年の 10 年間に心臓手術を受けた一連のすべての症例を研究対象集団とした。コホート内症例対象研究により、SSI および中心静脈カテーテル関連感染のリスク因子を特定した。症例は中心静脈カテーテル関連感染後に SSI を発症した患者とし、対照は中心静脈カテーテル関連感染後に SSI を発症しなかった患者から無作為に選択した。合計で 7,557 例の患者を登録し、133 例（1.7％）に SSI が確認された。このうち表在性 SSI は 0.7％（95％信頼区間［CI］0.5％ ～ 0.9％）、縦隔炎は 1.0％（95％CI 0.8％ ～ 1.2％）であった。SSI 症例 133 例中 12 例（9.0％、95％CI 5.0％ ～ 14.8％）は、中心静脈カテーテル関連感染後に同一の微生物により発症した。中心静脈カテーテル関連感染（補正オッズ比［aOR］5.2、95％CI 3.2 ～ 8.5）、冠動脈バイパス術（aOR 2.9、95％CI 1.6 ～ 5.2）、および肥満（aOR 11.4、95％CI 1.0 ～ 130.1）が、SSI と関連する独立因子であった。本研究の新たな知見として、中心静脈カテーテル関連感染を有する患者のSSI 発症率は、有していない患者と比較して 5.2 倍であった。

監訳者コメント：
手術部位感染症（SSI）の発生と中心静脈カテーテル関連感染の関係を疫学調査した論文であるが、肥満や冠動脈バイパス術がエントリーされているため、高血糖などの別なリスク因子の検討が必要である。
