病院内の手術部位感染（SSI）発生のモニタリングおよび報告を支援するために専用に開発されたソフトウエアについて述べる。本ツールでは、ロンドンの教育病院で感染症サーベイランスシステムの一環として日常的に収集されているデータを使用する。これらのデータには退院後追跡調査が含まれる。これらのデータを基に、本ツールを用いて経時的な感染症累積症例数を示すグラフ、および診療科ごとの感染症リスクの予測平均値を示す variable life-adjusted display（VLAD）チャートを作成する。ユーザーはグラフ作成に際し、その他のオプションとともに、使用する感染症の定義を選択することができる。本ツールとその使用目的について、整形外科手術における SSI のモニタリングを実例として示すが、その趣旨は明確な結論を提示することではなく、さらなる検証の契機とすることである。本ツールは各診療科に討論の材料を提供することになり、最終的には手術患者の安全性の向上をもたらすべきものであることを示す。著者らは、手術部位のサーベイランスを継続的に実施しているあらゆる医療機関に対して、本ツールおよび VLAD チャートを採用することを推奨する。

監訳者コメント：

SSI の累積発生状況の経時的変化に関し、リスク調整されたデータを提示できるという点でこのツールは非常に有用であると考える。一方で、SSI の判定をアルゴリズムを用いている点については、正確さに若干不安が残る。SSI の判定は、電子的診療情報だけでは必ずしも行うことができないからである。また、病院の電子情報システムとのリンクが必要など、課題も多い。これらの点は、情報技術の進歩で解決されるかもしれない。
