Do contact precautions cause depression? A two-year study at a tertiary care medical centre

接触予防策は感染症伝播の減少を目的として実施されるものであり、入室時のガウンや手袋の着用もこれに含まれる。これまでの小規模な研究から、接触予防策とうつ病や不安の症状悪化との関連が示唆されている。本研究では、2 年間に 3 次医療施設に入院した全患者の後向きコホートを対象として、接触予防策とうつ病や不安との関連を評価した。2 年間で 70,275 件の入院があり、このうち非集中治療室（ICU）かつ非精神科への初回入院は 28,564 件であった。この非 ICU 患者集団では、潜在的交絡因子を補正後、接触予防策はうつ病と関連していた（オッズ比［OR］1.4、95％信頼区間［CI］1.2 ～ 1.5）が、不安とは関連していなかった（OR 0.8、95％CI 0.7 ～ 1.1）。すなわち、接触予防策が実施された一般入院患者では、うつ病の有病率が40％高かった。

監訳者コメント：

接触予防策は感染対策に有効であるが、その弊害も指摘されている。本研究ではうつ病との関連が示されたが、スタッフの訪室が減ることによる観察頻度の減少により、転倒などのリスクが高まることが指摘されている。感染対策とその他の有害事象との両立は難しい問題である。
