Sequential outbreaks of multidrug-resistant Acinetobacter baumannii in intensive care units of a tertiary referral hospital in Italy: combined molecular approach for epidemiological investigation

2007 年 1 月から 2010 年 5 月に、イタリア、ジェノヴァの 3 次病院である San Martino Hospital で臨床検査に基づくサーベイランス研究を実施し、5 か所の集中治療室（ICU）で多剤耐性アシネトバクター・バウマニー（Acinetobacter baumannii）の分子疫学を調べた。合計 53 株の A. baumannii を ICU 入室患者（69.8％）および疫学的に関連のあるその他の病棟の入院患者（30.2％）から分離し、repetitive extragenic palindromic PCR（REP-PCR）法、複数部位塩基配列タイピング（MLST）、および adeB 遺伝子配列タイピングにより遺伝子型を決定した。REP-PCR フィンガープリント解析、MLST、および adeB タイピングの結果には強い関連が認められ、流行の原因となった菌株を以下の 3 つの主要な流行型クローンに分類することができた。2007 年 5 月に初めて分離されたクローン A（REP-I/ST4、adeB-STII 遺伝子型）、2007 年 11 月から 2009 年 5 月に分離されたクローン B（REP-IV/ST95、adeB-STI 遺伝子型）、2008 年 7 月から 2010 年 5 月に分離されたクローン C（REP-VII/ST118、adeB-STII 遺伝子型）。MLST の結果から、流行型クローン A と C は広く蔓延するクローン複合体 CC92 に属し、関連があることが示された。カルバペネム耐性の遺伝的決定因子を調査したところ、クローン A では bla_OxA-58-like遺伝子とこれに隣接する ISAba2 および ISAba3 エレメントの存在、クローン B と C では bla_OxA-23-like 遺伝子とこれに隣接する ISAba1 エレメントの存在が、耐性と関連していた。分子的アプローチにより感染制御対策の迅速な導入が可能となり、広範な疫学的背景に基づくデータの評価を行うことができた。
