クロストリジウム・ディフィシル(Clostridium difficile)感染症患者の予後予測変数としていくつかのものが提案されているが、医療資源の利用の程度を予測する臨床的に有用なツールについては不明である。基礎疾患に基づく重症度スコアである Horn 指標は C. difficile 感染症リスクの高い患者を正確に予測する。本研究の目的は、臨床的および医療経済的転帰が不良である C. difficile 感染症高リスク患者の層別化のための Horn 指標の使用について評価することである。C. difficile 感染症と診断された入院患者を 3 か月間前向きに追跡した。各患者の C. difficile 毒素検査陽性判定日の Horn 指標スコアを算出し、これを用いて転帰の変数(入院期間、死亡率、医療費)の相違を層別化した。C. difficile 感染症患者 85 例(男性 50%、白人 64%)を登録した。退院時死亡率は、Horn 指標スコア 1 ~ 2 で 0%、3 で 5%、4 で 50%であった(P < 0.001)。3 か月死亡率は、Horn 指標スコアが 1 で 0%、2 で 5%、3 で 17%、4 で 60%であった(P = 0.0004)。3 か月医療費の中央値は Horn 指標スコア 1 で8,585 ドル、2 で 12,670 ドル、3 で 29,077 ドル、4 で 68,708 ドルであった(P < 0.001)。Horn 指標スコア 3 ~ 4 の患者の入院期間(平均 33.4 日[標準偏差 33.3 日])は、1 ~ 2 の患者(平均 15.1 日[標準偏差 16.2 日])より有意に長かった(P = 0.001)。本研究から、Horn 指標は臨床的および医療経済的転帰が不良である C. difficile 感染症高リスク患者を特定するための簡便かつ有用な方法であることが示された。

監訳者コメント

C. difficile による感染症を発症しやすいリスク因子と死亡するリスク因子との関連について、さらなる検討が必要であろう。
