Is closure of entire wards necessary to control norovirus outbreaks in hospital? Comparing the effectiveness of two infection control strategies

英国の病院におけるノロウイルス制御の標準的なアプローチでは、健康保護局の指針に記載されているように、感染症が発生した病棟の早期閉鎖を行う。これは、かつてランカシャー教育病院で実施されたことがある。しかし、この方法は病床使用日数の損失や入院のキャンセルなどの大きな影響を及ぼす。2008 年、当病院に新しい戦略が導入された。その要点は、感染症が発生した区画（病棟ではない）の閉鎖、区画へのドアの設置、清掃の強化、院内での迅速な分子的検査、および感染制御チームの強化等であった。これらの変更の影響を、新規戦略導入の前後 2 回のノロウイルスシーズン（2007/2008 年と2009/2010 年）を比較することによって評価し、シーズン間の差を各シーズンの予測カウントの比（r）によって示した。確認された病院アウトブレイクの市中アウトブレイクに対する発生率の比は有意に低下し（r＝ 0.317、P ＝ 0.025）、アウトブレイクあたりの病棟への入院制限日数（r ＝ 0.742、P ＝ 0.041）およびアウトブレイクあたりの病床使用日数の損失（r ＝ 0.344、P ＜ 0.001）は有意に減少した。しかし、病院アウトブレイクあたりの感染患者数（r ＝ 1.080、P ＝ 0.517）、およびアウトブレイクあたりの感染医療従事者数（r ＝ 0.651、P ＝ 0.105）には有意な変化は認められなかった。ノロウイルスアウトブレイク時の病棟全体の閉鎖は必ずしも必要ではない。当病院で実施された変更によりアウトブレイクあたりの病床使用日数の損失が有意に減少し、同時にアウトブレイクの頻度の減少および大幅なコスト軽減がもたらされた。
