緑膿菌（Pseudomonas aeruginosa）は、集中治療室（ICU）における肺炎合併の重要な病原菌である。著者らの目的は、ICU の院内肺炎患者における緑膿菌肺炎の「患者側」および「病棟側」のリスク因子を明らかにすることである。2004 年から 2006 年に ICU を対象として実施されたフランスの院内感染症の全国的な前向きサーベイランス（REA-RAISIN）から、ICU に 48 時間を超えて入室し、院内感染肺炎を発症した患者のデータを使用した。初発肺炎のみを対象として、緑膿菌肺炎またはその他の細菌による肺炎のいずれかに分類した。緑膿菌肺炎を 2 値アウトカムとした多層ロジスティック回帰モデル（第 1 層：患者、第 2 層：病棟）による解析を実施した。201 の病棟の患者 3,837 例の 25％が緑膿菌肺炎であった。遅発型肺炎のほうが緑膿菌検出率が有意に高かった。緑膿菌肺炎の発生率が高いことは、高齢、人工呼吸器装着期間が長いこと、入院時の抗菌薬投与、病棟や ICU からの移送、および緑膿菌感染症の発生率が高い病棟への入院と関連していた。緑膿菌検出率が低いことは、外傷および病床回転率が高い病棟への入院と関連していた。本解析により、緑膿菌感染が疑われる院内肺炎症例の患者プロファイルおよび病棟側の要因が特定された。
