Laboratory diagnosis of Clostridium difficile infection in Spain: a population-based survey

スペインの 103 の病院を対象として、クロストリジウム・ディフィシル(Clostridium difficile)感染症の臨床検査診断に関する調査を実施した。C. difficile 感染症検出のため、入院 1,000 件あたり平均 23.2 の便検体の検査を行った。C. difficile 毒素検査による診断のために便検体の選別を実施している検査室は、全体の 35.9%であった。選別基準として多く用いられていたのは、軟便または水様便、抗菌薬療法歴、および院内感染下痢症であった。大半の検査室(95.1%)は自施設内で便検体の検査を行っており、その頻度はほとんどが週 5 ~ 7 日であった。1 施設を除くすべての検査室は便検体からの直接的な毒素検出を行っており、94.9%の検査室が酵素免疫測定法(EIA)を使用していた。診断法として毒素産生性検査培養を使用していた検査室は 16.3%のみであり、その大半は EIA を用いて分離株の毒素検出を行っていた。最も頻度が高い診断法は EIA 単独による便検体からの毒素検出であり(79.6%)、次いで便検体と分離株からの毒素検出を組み合わせる方法であった(10.2%)。C. difficile 感染症の 1 年間の推定発生率は入院 1,000 件あたり 1.71 件であり、大規模病院で有意に高値であった。スペインでは、ほとんどの検査室が EIA による便検体の直接的な検査を唯一の診断法としているため、C. difficile 感染症は過小診断されている。検査室を対象とした C. difficile 感染症の診断法および診断のための検査プロトコールに関する全国レベルの調査は、簡易かつ低コストであり、C. difficile 感染症の至適な診断のための国レベルでの基準の策定に向けた第一歩となると考えられる。

我が国でも国内の疫学に関する情報に乏しいため、こうした取り組みが今後は必要となろう。
