クロストリジウム・ディフィシル(Clostridium difficile)の診断検査:英国の臨床検査室を対象とした包括的調査

最近の研究から、クロストリジウム・ディフィシル(Clostridium difficile)感染症の臨床検査に広く用いられている毒素検出のための酵素免疫測定法(EIA)の性能が低いことが示されている。2009 年から 2010 年に、英国健康保護局と欧州臨床微生物学・感染症学会(European Society of Clinical Microbiology and Infectious Diseases;ESCMID)は、毒素 EIA 検査単独では不十分であるとの提言を行い、2 段階の検査プロトコール(1 つの方法でスクリーニングを行い、別の方法で結果を確認する)を推奨した。英国国民保健サービス(NHS)の全急性期トラストの検査法と陽性率を明らかにするため、情報公開請求による調査を実施した。170 のトラスト中 168 から回答を得た(回答率 99%)。70%のトラストは毒素 EIA を単独の検査法として用いていたが、その一部では陽性的中率(PPV)がわずか 20%であった。平均陽性率は 2008 年の 6.45%から 2009 年の 4.47%へと低下したが、このことはこれらの検査の PPV に負の影響を及ぼすことが予想される。英国保健省は、C. difficile 感染症発生率はケアの質および適正な感染制御の実践の指標であるとしている。しかし、検査法に大きなばらつきがあるため、C. difficile 感染症発生率に基づいて適切な比較を行うことはできない可能性がある。本研究は英国の C. difficile 感染症の検査法には大きなばらつきがあることを示しており、臨床検査室はそれぞれの現行の検査戦略を再検討すべきである。
監訳者コメント:
C. difficile のスクリーニング検査には、グルタミン酸塩脱水素酵素(glutamate dehydrogenase;GDH)とトキシンAならびにBが採用されている。培養法と併せても100%にはならない。もっとも、発症している場合と保菌している場合を検査で区別できるわけではないので、臨床症状の評価が重要である。米国では遺伝子増幅検査法が台頭している。
