Nosocomial rotavirus gastroenteritis in a Canadian paediatric hospital: incidence, disease burden and patients affected

ロタウイルスは小児科における院内感染病原体としてよく知られている。ロタウイルス胃腸炎はワクチンで予防可能な疾患であるが、カナダには現在、公的資金で運営されるプログラムは存在していない。本研究の目的は、院内感染ロタウイルス胃腸炎の発生率を明らかにするとともに、疾患負荷を推計し、患児の特性を明らかにすることにより、ロタウイルスワクチン接種戦略に関する情報を提供することである。カナダの 3 次小児病院で 10 年間に特定された院内感染ロタウイルス胃腸炎患児全例を対象とした後向きコホート研究を実施した。症例（214 例）の特定は、当院の院内感染の前向きサーベイランスプログラムにより行った。発生率は 1,000 患者日あたり 0.5 件（95％信頼区間 0.43 ～ 0.57）であり、この 10 年間で有意な減少は認められなかった。また発生率は、入院日数が 5 日を超える患児は有意に高かった。慢性基礎疾患が 126 例（59％）に認められたが、その有病率は入院歴がある患児では高く、また 95 例（44％）は生後早期に診断されていた。132 例（62％）は輸液が必要となり、このうち 98 例（46％）に静注輸液が行われた。26 例（12％）は退院後に発症した院内感染ロタウイルス胃腸炎により再入院が必要となり、入院日数中央値は 4 日であった。小児病院では依然として、ロタウイルス院内感染は主に再入院や長期入院を要する基礎疾患を有する小児の重要な問題である。カナダやロタウイルスワクチンの全員接種制度が導入されていないその他の国では、先天性疾患のある乳児や低出生体重児などの高リスク患児を対象としたロタウイルスワクチン接種プログラムの評価を実施する必要がある。
