高齢者療養施設の看護師の空中レジオネラ菌によるポンティアック熱

本研究の目的は、高齢者療養施設の入所者の入浴介助を行う看護師を対象として、ポンティアック熱に認められる症状の発生率を明らかにするとともに、レジオネラ菌への曝露との関連を評価することである。ポンティアック熱の非流行下で、高齢者療養施設 19 か所の看護師 104 名を平均 4 か月間追跡した。各対象者が症状、入浴介助回数、および使用したシャワーの位置についてデータを記録した。追跡終了時に、水中および空中の細菌の特性を培養および in situ ハイブリダイゼーション法により評価した。ポンティアック様エピソード 11 件中 8 件が本研究のポンティアック熱の定義に合致した。レジオネラ菌の水中濃度が 1 L あたり 103 cfu を超えることがポンティアック熱のリスク増加と関連しており、リスクは濃度依存性であった。レジオネラ菌の空中濃度とポンティアック熱イベントとの関連は確認されなかった。レジオネラ菌が存在する労働環境からの保護を考慮する場合に、レジオネラ菌水中濃度 1 L あたり 103 cfu という閾値を利用することができると考えられる。
