Burden of healthcare-associated infections in a paediatric intensive care unit of a developing country: a single centre experience using active surveillance

医療関連感染（HAI）は、あらゆる年齢層の重症患者における合併症および死亡の重要な原因である。この前向きサーベイランス研究の目的は、発展途上国の小児集中治療室（PICU）における HAI による負荷の推定である。12 か月の研究期間中に、患児 187 例が PICU で 48 時間以上の治療を受け、このうち 36 例に 44 件の HAI エピソードが認められた。HAI の粗発生率は患児 100 例あたり 19.3 例、発生密度は 1,000 患者日あたり 21 例であった。HAI の 44事例中 27 件（61％）は医療関連肺炎、12 件（27％）は医療関連血流感染、4 件（9％）は尿路感染であった。HAI を発症した患児は平均入院期間が有意に延長し（25 日対 7 日、P ＜ 0.0001）、死亡率が有意に高かった（50％対 27.8％、P ＜ 0.005）。医療関連肺炎および医療関連血流感染の最も頻度が高い原因菌はアシネトバクター（Acinetobacter）属菌であった。発展途上国では、高リスク集団の HAI の負荷を減少させるうえで積極的監視培養が重要である。
