本研究では、抗菌薬処方および電子データの点有病率調査に基づく医療関連感染症の有病率に関して、以前報告した方法をデバイス関連感染症有病率の推計に応用した。6 か月ごとの抗菌薬処方の点有病率調査を、2009 年 6 月に欧州抗菌薬使用プロトコールサーベイランス(European Surveillance of Antimicrobial Consumption Protocol)に従って実施した。抗菌薬投与を受けている患者についてはデバイスの有無を記録した。同時に、関連病院でのデバイス使用状況の調査を実施した。対象は、入院後 48 時間以降に抗菌薬投与を開始した患者で、デバイスを使用しているか、デバイスは使用していないが血流感染症、尿路感染症、または肺炎の治療のために抗菌薬投与を受けていることを条件とした。微生物検査または画像検査の結果が陽性であることが報告されている患者を特定し、デバイス関連感染症の有無を事前の定義に従って評価した。調査対象患者 1,354 例中 253 例(19%)が抗菌薬治療を受けていた。このうちデバイスも使用しているのは 189 例であり、デバイス関連感染症の有無を個別に評価する必要があったのは 172 例(全調査対象患者の 13%のみ)であった。微生物検査または画像検査の電子報告書の確認に要する時間は、患者 1 例あたり約 5 分であった。23 例がデバイス関連感染症の基準に合致した。有病率は、カテーテル関連尿路感染症 3.9%、中心ライン関連血流感染症 3.1%、血管アクセス部位感染症 3.8%、人工呼吸器関連肺炎 11.6%であった。本法は、既存データを用いてデバイス関連感染症の有病率を推計するための簡便な方法であり、他の病院でも実施可能である。
