多剤耐性緑膿菌(Pseudomonas aeruginosa)アウトブレイク時の遺伝子型および感染部位★

日本の急性期病院で多剤耐性緑膿菌(Pseudomonas aeruginosa)のアウトブレイクが発生し、3 年以上持続した。2006 年 1 月から 2009 年 6 月に入院患者 59 例から多剤耐性緑膿菌が検出され、アウトブレイク前半は主として尿培養から、後半は気道サンプルからの分離が優勢となった。重複のない 51 例からの多剤耐性緑膿菌分離株が評価可能であり、いずれも blaVIM-2 陽性であった。パルスフィールド・ゲル電気泳動(PFGE)法により分離株は 3 つの主要なクラスターに分類され、8 株が A 型、19 株が B 型、21 株が C 型であった。アウトブレイクは PFGE A 型株を保有する患者から始まり、次いで B 型、さらに C 型が続いた。多変量解析から、PFGE C 型株は気道サンプルからの検出が多いことが示された(オッズ比 11.87、95%信頼区間 1.21 ~ 116.86)。無菌的尿路カテーテル管理の改善により A 型および B 型株は制御され、気道のケア手技の改善により最終的に C 型株の伝播も制御された。
監訳者コメント:
耐性菌の遺伝子型(PFGE)と検体の種類との関連を研究し、大変興味深い結論となっている。日本の研究者達によって行われた、対策につながる分子疫学的調査といえる。
