Klebsiella pneumoniae producing KPC carbapenemase in a district general hospital in the UK

多剤耐性 KPC 型カルバペネマーゼ産生肺炎桿菌（Klebsiella pneumoniae）による尿路感染症を認めた患者 2 例について報告する。PCR 法およびシークエンシングにより、両患者の分離株から bla_KPC-2 遺伝子が検出された。これらの分離株はパルスフィールド・ゲル電気泳動パターンが一致しており、MLST 解析では ST11 型であった。初発症例はキュラソー島の病院に入院中に KPC 産生株を獲得し、その後当院で 2 例目の患者に院内伝播したと考えられた。急性期トラストおよび地域でのさらなる伝播を予防するために実施した介入について述べる。

カルバペネム耐性をもたらす遺伝子のうち、KPC はクラス A に属し、KPC 産生の肺炎桿菌や大腸菌などによる医療関連感染が近年、懸念されつつある。アメリカやヨーロッパの一部の国では頻繁に分離されるが、それ以外の国や日本ではまだあまり分離されない。本論文は、比較的分離が少ないと思われるイギリスの病院での 2 症例が記述されており、日本の病院もこのような状況に遭遇する施設が今後、増加するものと思われる。外国や他地域での医療受診歴が本菌のリスク因子として注目されるべきである。
