Funnel plots and risk-adjusted count data adverse events. A limitation of indirect standardisation

病院の有害アウトカムデータの報告の義務化が進んでいることから、一部の医療施設ではこれらのデータの解析・表示方法の検討が促されている。現在、多くの研究者が好む方法は漏斗プロットである。このプロットの作成には間接的標準化が行われることが多い。手術部位感染症や死亡などのバイナリ・データの表示には、この方法で十分であると考えられる。これらのデータはリスク補正が行われることが多くなっている。これらのデータのリスク補正では通常、同一または類似の処置を受けた個々の患者が対象であるため、この方法が誤謬につながることはないと思われる。一方、菌血症などの度数データを扱う場合にデータのリスク補正を行うと、この方法は誤謬をもたらすことがある。その理由は、情報として個々の患者ではなく、病院や診療科を単位としたデータを使用するからである。例えば、形成外科や心臓外科のように敗血症の発生が少ない診療科を擁する病院がある一方、腎不全の治療（移植を含む）を行う部門や大規模な血液・腫瘍科（移植も含む）などの菌血症の発生率が高いことが予想される診療科を設けている病院もある。さらに、病院および病院の診療科の規模は大幅に異なっていることがある。分母が大きく異なると、間接的標準化により結果にバイアスが生じ得ることはよく知られている。本稿では、Queensland Health Centre for Healthcare Infection, Surveillance and Prevention（CHRISP）データベースのリスク補正後の菌血症データを用いて、この問題を解説する。
