中国湖北省の集中治療室における人工呼吸器関連肺炎:多施設前向きコホート研究

Ventilator-associated pneumonia in intensive care units in Hubei Province, China: a multicentre prospective cohort survey

中国湖北省の 3 次病院の 17 の集中治療室(ICU)を対象として多施設前向きコホート研究を実施した。人工呼吸器関連肺炎(VAP)の定義は、既報の文献の改訂基準に従った。人工呼吸器を使用した患者 4,155 例の VAP の粗発生率は 20.9%、発生率は 1,000 人工呼吸器日あたり 28.9 例であった。ロジスティック回帰を用いた多変量解析により、リスク因子は男性であること(リスク比[RR]1.5、P < 0.001)、昏睡(RR 2.1、P < 0.001)、慢性閉塞性肺疾患(RR 1.4、P < 0.001)、他の部位の感染症(RR 1.6、P = 0.001)、VAP 発症前の重篤な疾患(RR 1.6、P < 0.001)のほか、制酸薬投与(RR 1.4、P < 0.001)、抗菌薬投与(RR 5.1、P < 0.001)、気管支鏡検査(RR 1.5、P = 0.041)、気管切開(RR 1.4、P = 0.014)などの介入であることが明らかとなった。分離頻度が高い起因菌は、緑膿菌(Pseudomonas aeruginosa)およびアシネトバクター・バウマニー(Acinetobacter baumannii)であった。すべての黄色ブドウ球菌(Staphylococcus aureus)分離株のうち 45.7%がメチシリン耐性であった。湖北省の ICU における VAP の発生率、リスク因子、および起因菌は、先進国からの報告とは異なる。これらのデータは、中国湖北省ではより効果的な感染制御のための介入が必要であることを示している。
