病院環境の消毒と感染制圧のための過酸化水素の空中散布:システマティックレビュー★

環境の消毒薬および臨床環境における感染制圧策としての過酸化水素の空中散布の有効性について文献レビューを行った。システマティックレビューにより対象に適格な 10 件の研究を特定した。過酸化水素は、7 件の研究では蒸気として、3 件の研究はドライミストとして散布されていた。評価の対象となった病原体は、5 件の研究でメチシリン耐性黄色ブドウ球菌(meticillin-resistant Staphylococcus aureus;MRSA)、3 件でクロストリジウム・ディフィシル(Clostridium difficile)、および 2 件では複数の細菌種であった。該当データを個別に報告している 9 件の研究では、何らかの清掃介入を実施する前に採取した合計 480 か所の環境からのサンプルの 187 か所(39.0%、範囲 18.9% ~ 81.0%)が、研究の対象とした病原体に汚染されていた。6 件の研究では、最終清掃後に採取した 630 か所のサンプルの 178 か所(28.3%、範囲 11.9% ~ 66.1%)、10 件の研究では過酸化水素散布後に採取した 682 か所のサンプルの 15 か所(2.2%、範囲 0% ~ 4.0%)に汚染が残存していた。4 件の研究は、感染制圧のための過酸化水素蒸気の使用について評価を実施していた。この中の 2 件の研究では過酸化水素蒸気の散布により院内アウトブレイクが制圧され、残りの 1 件では MRSA による持続的な環境汚染が根絶されて、1 件では C. difficile 感染症が減少していた。

監訳者コメント:
過酸化水素蒸気による環境消毒は、カナダや米合衆国において、深刻な高病原性 C. difficile の流行への対策として開発が進んだ背景があり、その他にも医療環境の汚染が重要な位置を占める多剤耐性アシネトバクター・バウマニー(Acinetobacter baumannii)への対策としても注目されている。ただし、実際に臨床的な効果が認められるか実証的な研究を積み重ねなければならないと考える。
