Newcastle upon Tyne Hospitals NHS Foundation Trust(NuTH)は 2006 年、患者のメチシリン耐性黄色ブドウ球菌(meticillin-resistant Staphylococcus aureus;MRSA)保菌・感染を最小限に抑えるために「探し出して、粉砕する」(seek and destroy;S&D)プログラムを開始した。段階的に導入し、2008 年 9 月までにすべての診療科を計画に組み入れたが、これは英国保健省による全患者のスクリーニングの義務化よりかなり以前のことである。NuTH は発色性培養法(chromogenic culture method)により 1 か月あたり約 15,000 例の鼻、咽喉、および会陰部の試料のスクリーニングを行い、MRSA 陽性率は平均 2.4%であった。微生物・感染管理サービスを週 7 日体制で提供することにより、除菌治療の処方までに要する時間を24時間未満とした。168,073 の試料の解析結果から、MRSA の検出率を向上させるためには、3 か所のスクリーニング部位すべてを対象とする必要があることが判明した。低リスク領域の患者をスクリーニングプログラムの対象とすることを必須とされたため、作業量は大幅に増加したものの、MRSA 検出率は向上しなかったことが、S&D に基づく手法の評価から示された。典型的な 1か月間のデータのレビューから、我々の標的を絞った S&D アプローチによっていれば MRSA 保菌者であることが特定できなかったであろう患者数は、英国保健省のユニバーサルスクリーニングとの比較で、日帰り入院の 7 例のみであった。これらの患者を追加的に検出するために要する検査費用の総額は 1 か月で 20,000 ポンドであり、これによって追加的に発生する MRSA 陰性患者は 4,200 例であった。本研究から、臨床的リスクに基づくスクリーニング戦略は、イングランドで現在求められているユニバーサルプログラムよりも実用的かつ費用対効果が高いことが示唆される。

監訳者コメント
イギリスで始まった全患者に対する義務的 MRSA スクリーニングに対する批判的論文。MRSA のスクリーニングは、疫学的状況に応じてその実施方法を考えるべきであり、まっとうな意見を述べている。このような論文が国の方針に今後どのような影響を与えるか、見物である。
