World Health Organization-recommended hand-rub formulations do not meet European efficacy requirements for surgical hand disinfection in five minutes

世界保健機関（WHO）は、使用施設で調製する 80％エタノールまたは 75％イソプロパノール（いずれも v/v）ベースの 2 種類の擦式製剤を推奨している。EN 12791 に準拠して、これらの有効性を評価した。被験者 26 名が標準処置（n-プロパノール、60％）による 3 分間の手指消毒、または 2 種類の製剤のいずれかによる 1.5、3、または 5 分間の手指消毒を行った（ラテン方格法）。処置後細菌量（即時効果）を一方の手指で測定し、対側の手には手袋を 3 時間装着した。手袋を取り外した後、二次的処置後細菌量（3 時間後の効果）を測定した。各擦式製剤の全 3 種類の処置時間による平均減少量の対数値（log₁₀）を、Hodges-Lehmann 検定を用いて比較評価した。第 1 ブロックでは、標準処置により細菌量が 2.43 log₁₀（即時効果）および 2.22 log₁₀（3 時間後の効果）減少した。エタノール製剤の有効性（例えば、5 分間処置後の即時効果は 1.41 log₁₀ の減少）は、いずれの処置時間でも標準処置に対して劣性が認められた（95％信頼区間［CI］下限値 ＜ －0.75）。第 2 ブロックでは、標準処置により細菌量が 2.72 log₁₀（即時効果）および 2.26 log₁₀（3 時間後の効果）減少した。イソプロパノール製剤の有効性（例えば、5 分間処置後の即時効果は 2.05 log₁₀ の減少）についても、いずれの処置時間でも標準処置に対して劣性が認められた（95％CI 下限値 ＜ －0.75）。WHO が推奨する擦式製剤はいずれも、5 分以内の手術時手指消毒に関する EN 12791 の有効性の要件に合致していなかった。有効成分の濃度を高めることにより、この有効性が改善する可能性がある。
