尿路感染症発生率のベンチマーキング:ドイツ全国院内感染症サーベイランスシステムの集中治療部門コンポーネントから得られた経験★
Benchmarking of urinary tract infection rates: experiences from the intensive care unit component of the German national nosocomial infections surveillance system

この研究の目的は、集中治療部門(ICU)における症候性のカテーテル関連尿路感染症(CAUTI)サーベイランスにより、CAUTI 発生率が低下するかどうかを調べることである。ドイツ全国院内感染症サーベイランスシステム(KISS;Krankenhaus Infektions Surveillance Systems)のデータを、3 通りの開始時期(1997 ~ 2000 年、2001/2002 年、2003 年以降)から収集して、解析対象とした。各期間の 1 年目と 3 年目に得たデータを比較した。CAUTI 総発生率を参加期間に毎月算出して、線形回帰モデルをあてはめた。1997 年 1 月から 2008 年 6 月まで、合計 547 か所の ICU が KISS の ICU コンポーネントにデータを提供した。研究プロトコールに従って、267 か所の ICU における 1,966 例の症候性 CAUTI 症例を解析対象とした。症候性 CAUTI 発生率を 3 年目と 1 年目において比較したところ、全相対リスクは 0.86(95%信頼区間 0.77 ~ 0.96)であった。CAUTI 発生率に対するサーベイランスの影響は、人工呼吸器関連肺炎や原発性血流感染症に関する同様のデータと比較して極めて小さかったが、これは CAUTI に対する多くの集中治療専門医の認識が、他の 2 種類の感染症と比較して欠如しているためであると考えられる。しかし、症候性 CAUTI の減少を図ることは可能であり、CAUTI をすべての ICU サーベイランス活動の対象に含めたとしても、作業負荷が大幅に増加することはない。

提示された KISS のデータによれば、集中治療部門(ICU)における症候性カテーテル関連尿路感染症の発生率は、のべ 1,000 カテーテル・日あたり 2000 年以前が 1.39、2001/2002 年が 0.83、2003 年以降が 0.68 となっており、有意な減少傾向を示している。国家的なサーベイランス事業が継続的医療質改善に資するデータを提供している。
