Determination of antiseptic efficacy of rubs on the forearm and consequences for surgical hand disinfection

手指は外科チームの皮膚における病原巣として最も重要なものであると認識されているが、前腕からの病原体の伝播も関与している可能性がある。術前の手指消毒が推奨されているが、前腕についてはエビデンスに基づく標準的な消毒法はない。欧州標準 EN 12791および米国のガイドライン ASTM1115 はいずれも前腕を対象としていないため、欧州標準 EN 12791 およびドイツ衛生微生物学会（German Society for Hygiene and Microbiology；DGHM）の皮膚消毒薬の有効性の検査法に基づいて、前腕を対象とした新しい検査法を開発した。市販の擦式アルコール製剤（76.7％［w/w］エタノール）の消毒効果を、上腕については 15 秒、2.5 分、30 分後、前腕については 2.5 分、30 分、3 時間後に評価した。上腕への製剤の塗布は DGHM の標準的手順に従った。前腕への製剤の塗布は、術前の手指消毒のように被験者自身が右手指で左前腕に塗布し、逆も同様とした。試料の採取および培養を、DGHM による上腕の皮膚消毒法に従って実施した。ボランティア 22 名を対象とした。前腕の消毒薬処置の効果は、いずれの検査部位についても、上腕と比較して有意に低いということはなかった（P ＞ 0.05）。いずれの検査部位および消毒後時間においても、減少係数（reduction factor）は極めて近似しており、95％信頼区間の下限は 1.43 log₁₀、上限は 2.31 log₁₀ であった。適切な製剤を使用する場合には、術前手指消毒の一環としての前腕の処置は 10 秒の塗布時間で十分であると考えられる。

監訳者コメント：
確かに、古典的外科手洗いでは肘までスクラブ剤で処理するのに対して、これらの指針では前腕部をカバーしていない。結論として、前腕は手指より菌量が少なく、手指の対応よりも緩やかでも十分な効果が得られている。
