Intraoperative technique as a factor in the prevention of surgical site infection

手術を受けた患者の約 5％が手術部位感染症（SSI）を発症する。SSI 患者は入院期間が 7 日間延長し、術後死亡率が上昇する。SSI の予防には、外科医の能力と技術が重要であると考えられる。著者らは、術中手技の種々の特性にかかわるエビデンスと、SSI 予防に対するその意義について調査した。ジアテルミーによる皮膚切開は切開時間の短縮および出血量の減少をもたらすため、感染症発生率の低下に寄与するが、英国国立医療技術評価機構（NICE）の最新のガイドラインはジアテルミーを避けることを推奨している。連続縫合と結節縫合など、各種閉創術の比較研究では、SSI 発生率に統計学的有意差は認められていないが、連続縫合は所要時間が短い。汚染創は、一次治癒が生じる前に局所感染症の治療ができるように、手術創を 4 日間開放しておくべきである。手術時ドレーンは手術創とは別の切開創に設置すべきであり、閉鎖式吸引ドレーンのほうが開放式ドレーンより望ましく、またすべてのドレーンは可及的速やかに抜去すべきである。SSI 発生率への術中手技の影響に関する大規模試験は比較的少ない。術中手技のどのような特性が SSI に影響するのかを明らかにするため、また手術の研修者を対象とした手術実務の情報提供および教育プログラム支援のため、さらに大規模な多施設前向き試験が必要である。
