Determination of factors required to increase uptake of influenza vaccination among hospital-based healthcare workers

病院に勤務する医療従事者のインフルエンザワクチン接種遵守の予測因子を明らかにするために、オランダの 8 つの大学医療施設を対象として質問票による研究を実施した。行動モデルおよび実践モデルに基づいて、人口統計学、行動、および組織に関連する決定因子の評価を行った。多変量回帰分析により、インフルエンザワクチン接種の独立予測因子を評価した。年齢 40 歳超、慢性疾患の存在、個人的リスクの認識、他の患者への感染リスクの認識、ワクチンが他の患者への感染リスクを低下させる効果を信頼していること、医療従事者は周囲に害を与えてはいけないという義務感、医療従事者は医療行為の継続を確実に行うべきという義務感、外部から常に人が院内に入ってくるにもかかわらずワクチン接種は有効であるという知識、オランダ健康審議会（Health Council）の勧告についての知識、社会からの影響、都合のよい時間にワクチン接種が実施されることは、いずれもワクチン接種と独立した関連があった。予測モデルの精度は極めて高かった（AUC ＝ 0.95）。インフルエンザワクチン接種を受ける医療従事者の増加を図るための介入プログラムでは、本研究で特定された決定因子を標的とするべきである。

監訳者コメント：
医療従事者のインフルエンザワクチン接種率は、日本の医療施設において近年めざましい向上がみられる。背景には、その費用負担を主に医療施設としていること、日本人が組織の取り決めにあまり抵抗せず（疑いもせず）従う傾向にあること、などがあげられよう。これに対して欧米では接種率がなかなか向上しないという報告をよく目にする。本論文でも 1,300 名の回答者における接種率は 37％にとどまっている。その分、ワクチン接種と非接種の群における要因の相違に関する検討は容易であり、本論文ではそれが詳細に行われている。すでに高い接種率を得ている医療施設ではあまり参考にならないだろうが、これから接種率向上を目指す施設では参考になる結果である。
