Involving the patient to ask about hospital hand hygiene: a National Patient Safety Agency feasibility study

英国では毎年 300,000 例以上の患者が医療関連感染症に罹患する。医療関連感染症は医療システムの重大な負荷となっているが、その多くは予防が可能である。医療従事者の手指衛生は医療関連感染症の主要な予防法であるが、適切な手指衛生実践の遵守率は低い。英国患者安全機構（National Patient Safety Agency）は一般市民、入院患者、および医療従事者、特に急性期病院5施設の第一線の臨床部門のスタッフや感染制御看護師を対象として、患者の関与や取り組みの程度を高めることが手指衛生遵守の向上と医療関連感染症の減少に寄与するという考え方に同意するかどうかを調査した。一般市民の 57％（530 名中 302 名）は、医師はすでに手指衛生を実践していると考えられるため、医師に手指の清潔度について質問することはないであろうと回答した。入院患者の 43％（210 名中 90 名）は、医療従事者は手指を清潔にすべきことを認識しているはずであると考え、実践していることを信じると回答し、20％（210 名中 42 名）は、業務を適切に実践する医療従事者の能力を自分が疑問視しているように思われたくないと回答した。調査対象の医療従事者の多く（254 名中 178 名）は、患者との接触前に手指衛生を実践したかどうかを質問されれば、医療関連感染症は程度の差はあっても減少するであろうと回答した。患者と医療従事者のいずれもが、各患者に擦式アルコール製剤入り容器を支給することによって、患者による医療従事者に対する手指衛生への注意喚起を促すこと、また「どうぞ質問してください」という姿勢を積極的に支援することが、最も有用な介入であると考えていた。しかし、このような患者の関与は医師（または医療従事者）と患者との良好な関係を損なうという医療従事者の誤解を正すには、さらなる研究が必要である。
