Hospital-wide survey of the use of central venous catheters

中心静脈カテーテル（CVC）の使用の適応に関するデータは、ほとんど報告されていない。留置期間中のカテーテル留置の適応を定量的に評価すること、および CVC の使用に関する医療従事者間での同意について調査することを目的として、全病院的な前向き観察コホート研究を実施した。カテーテル使用の観察を、医療機関への視察、医療従事者からの聞き取り調査、および患者カルテの調査により行った。CVC の挿入は患者 292 例に対して合計 378 回実施され、のべ留置期間は 2,704 カテーテル日であった。CVC の 93％はマルチルーメンカテーテルであり、70％は集中治療室（ICU）で留置されていた。留置期間中央値（および四分位範囲）は全病院 5 日（2 ～ 9 日）、ICU 4 日（2 ～ 7 日）、ICU 以外 8 日（3 ～ 15 日）であった。CVC 使用 1 日あたりの事前に規定した適応の平均件数は 1.7 件（ICU 1.9 件、ICU 以外 1.5 件、P ＜ 0.001）であった。最も頻度が高いカテーテル使用の理由は長期（＞7 日）の抗菌薬療法（49％）、次いで非経口栄養（22.3％）であった。不必要なカテーテル留置は合計 130 カテーテル日（4.8％）であり、ICU 以外のほうが高率であった（6.6％）。CVC の使用の適応について医療従事者間の同意が得られていた症例は 94％であった。しかし、ICU 以外の施設への視察のうち 35 回（8.3％）では、看護師および治療担当医師のいずれもカテーテル留置の理由を知らないことが判明した。ICU のほうがカテーテル留置期間は短いが使用頻度が高く、ICU 以外ではその逆であった。カテーテル使用を標的とする予防策は、ICU 以外の施設のほうが成功する可能性が高い。
