Tuberculosis screening programme using the QuantiFERON^R-TB Gold test and chest computed tomography for healthcare workers accidentally exposed to patients with tuberculosis

医療従事者の結核の発生率が上昇している。結核患者に接触した医療従事者の定期的および必要に応じたスクリーニングが重要である。著者らは、胸部 CT と QuantiFERON^R-TB Gold（QFT-G）検査を医療従事者に対する結核スクリーニングプログラムに組み入れた。最初に QFT-G 検査により接触者の検査を行った。QFT-G 検査陽性者に対しては CT 検査を実施し、活動性結核、潜伏性結核、または陳旧性結核に分類した。2005 年 4 月から 2010 年 4 月までの期間に、入院時に活動性結核との診断を受けなかった患者 11 例が活動性結核であることが判明した。濃厚接触者または高リスク接触者合計 512 名を特定し、スクリーニングを行った。このうち 34 名（6.64％）が QFT-G 陽性、478 名（93.36％）が陰性であった。QFT-G 陽性の医療従事者 34 名中 4 名は CT 所見で活動性結核が認められ、多剤併用療法を実施したが、24 名には活動性結核所見は認められずイソニアジドを 6 か月間投与した。全員が治療レジメンを完遂し、有害事象は認められなかった。CT と QFT-G 検査を組み合わせた結核スクリーニングプログラムは安全かつ適用可能であった。本プログラムの有効性を大規模臨床試験により確認する必要がある。
